Notice written by OB GYN Specialists of Lima: On the morning of June 14th, 2024, it was discovered that some Protected Health Information (PHI) was disposed of improperly. We attempted to retrieve it but were unable to retrieve all of it. If you had a visit during the dates of June 5th and June 13th or had any testing done in the prior 2 weeks you may have been affected.
Most of the information was demographic information that is printed when you have a visit. The remainder may have included test results coming into the office. There was no financial information included in the breach.
We have implemented a plan to prevent this from happening again. If you have any questions, feel free to contact the office at 419-227-0610.
